Client Grievance Form
The Client Grievance Form on the Accreditation Now website can be used to file an internal formal grievance. Please refer to your organizations complaint procedures, before filing a formal grievance report.
Mandatory fields include Date, Time, Location (if applicable), Name, Phone and Email. The Address fields are optional.
Describe your complaint in as much detail as possible.
Describe why you believe the action was wrongful, illegal, or unlawful.
Describe the resolution, or desired outcome, you are seeking.
Enter the name of a witness, when applicable, followed by the + symbol, to add more witnesses. Names may be removed by clicking the x symbol.
Enter and confirm a password to enable you to check back in and see the status of your report.
Submit.
